What will you do at my first visit?
Your initial appointment will consist of an examination and review of your treatment options. Sometimes treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. If you would like to have a cleaning at your first appointment, please discuss your scheduling options with our receptionist when you make your appointment. If you have special concerns, or need to see the doctor sooner than that, please let us know.
What Information will you need me to bring?
Please provide us with the following information at the time of your first visit:
Pre-Medication/Other special concerns
If you need to pre-medicate for any reason prior to your appointment please call our office prior to the date you are due to come in. In this case, you will need to complete your Acquaintance Form for New Patients prior to your visit so that we can call in a pre-medication for you.
Some reasons for taking a pre-medication include: heart murmur, heart valve replacement, or hip/knee/joint replacement. If you are unsure whether you need to take a precautionary pre-med, please call our office to discuss your situation with Dr. Dalton.
If you have any other special concerns that you would like to advise us of or discuss with us prior to your appointment, please feel free to call our office at any time.

- Most recent dental x-rays. If you would like us to request these records for you, please submit a copy of the Records Release Form to our office about a week prior to your appointment so we can request those records for you. Additional images can be taken in our office if necessary.
- A current list of your medications.
- Your dental insurance card, if applicable.
- Completed Acquaintance Form for New Patients.
